• Pressure injuries are never downgraded (eg from Stage 3 to Stage 2)
• Most ulcers occurs in the sacral and heel areas
• An ulcer with an unvisible wound base can never be staged
• Stable eschar (eg heel) are “biological covers” and should not be removed
• Even with the best preventive measures a pressure injury may still develop
